Readiness for Change Questionnaire

Readiness for Change Questionnaire

Before you can make meaningful change, one of the most important actions is measuring your "Readiness for Change". Understanding where you are in the "Stages of Change" will help you know what to do next. Assessing your readiness will help you to understand how willing you are to adjust your mindset, develop new habits, and change your current actions.

It's as easy as providing the most appropriate response to each of the following questions. Base your answers on your current situation. We’ll calculate your total score, and provide you with recommended next steps.

When looking at yourself in the mirror, do you feel upset, find yourself unrecognizable, or have a sense of frustration about what you see? *
What do you feel is the source of feeling tired or rundown? *
Are you taking any medications for heart disease, high blood pressure, or type II diabetes? *
Are you prepared to join a fitness center today? *
Would you listen if someone said you’d need to throw away food you have at home and shop for different foods more aligned with your goal? *
Are you open to speaking to your employer about changing work conditions, or finding new employment if your work environment presents significant barriers to you physical activity and nutrition? *
If you don’t have anybody to exercise with regularly, are you willing to look for a physical activity partner? *
If your fitness has deteriorated over the years, how do you explain the fact that you're in worse shape than when you were younger but haven’t changed your habits at all? *
